The SARS-CoV-2 virus, constantly changing, continues to circulate in population, causing reinfection. In this connection, a clinical case of viral bacterial pneumonia with septicopyemia development after SARS-CoV-2 reinfection is presented. Patient K., 44-years-old, was treated in department of infectious disease. He admitted with complaints of fever up to 38.1—38.4 °C, dry cough, chest pain, palpitation. Condition was assessed by NEWS2 scale and was equal to 11 scores. Computed tomography showed bilateral viral interstitial pneumonia. The RNA analysis of SARS-CoV-2 (COVID-19) was positive. Treatment was started, despite which the condition deteriorated, signs of acute cardiovascular and respiratory failure increased, and the result was fatal outcome. Final diagnosis: COVID-19 novel coronavirus infection, severe course. Complications of underlying disease: bilateral viral interstitial pneumonia. Acute respiratory distress syndrome. Sepsis. There was a pattern of viral bacterial pneumonia on the autopsy. Diffuse inflammatory infiltrates, dystrophic and necrobiotic alterations, contractural damage of myocytes, lumpy breakdown of myofibril, platelet and erythrocytic microaggregates in microvasculature, leukostasis were revealed in myocardium. Spleen size was unchanged, there was no flabbiness. Thus, the peculiarity of the case was the diffuse localization of the process in the myocardium, which is not found in sepsis, and the absence of macroscopic alterations in the spleen.
