Chronic pelvic pain syndrome (CPPS) in women is defined as persistent, noncyclic pain perceived to be in structures related to the pelvis and lasting more than six months. Very often no specific etiology can be identified. It is important to exclude other common causes associated with gynecological pathology (endometriosis, pelvic inflammatory disease and/or neuralgia of the pelvic organs and/or with other functional somatic pain syndromes (e.g., irritable bowel syndrome, nonspecific chronic fatigue syndrome) and mental health disorders (e.g., posttraumatic stress disorder, depression). This review provides key recommendations for practice and also considers the risk factors for CPPS, defines a variant of the algorithm for diagnosing this syndrome, the characteristics of pain syndrome and anamnestic data that allow one to suspect genital endometriosis, considers hormonal therapy for pathological conditions such as heavy menstrual bleeding (HMB) and endometriosis, as well as guidelines for the management of women with CPPS.
