Objective — to evaluate the grade of body mass reduction which leads to improvement of hormonal and metabolic state in women with obesity. Material and methods. The prospective study of 30 patients with obesity and menstrual disorders was performed. Results. Basal hyperinsulinemia was diagnosed in 26.6% women, postprandial hyperinsulinemia in glucose tolerance test was diagnosed in 100%. 90% had insulin resistance. Hypercholesterinemia was diagnosed in  53.3%, hypertriglyceridemia — in 23.3%, high concentrations of LDLP — in 6,6%. High LH level and LH/FSG >2.5 ratio was found in  16.6%, increased testosterone or/and androstendione — in 36.6%, 13.3% patients had high concentrations of cortisol and DHEAS. 2 (6,6%) women had reduced concentrations of SSBG. 26,6% women had normal hormonal status. Women were recommended to follow diet and to take 120 mg orlistat 120 mg 3 times a day with meals. After reducing of BMI on 2 kg/m2 (53%) women noticed that menstrual cycle became normal, ovulation was restored. After BMI reducing on 3 kg/m2 (86,6%) patients reported of normal cycle and ovulation. Improvement of lipid and carbohydrate metabolism was achieved after BMI reducing on 2 kg/m2. Reliable reduction of testosterone androstendion and growth of SSBG and IGF-1 concentrations were achieved after the same grade of weigh reduction. Leptin concentrations gradually raised from the beginning of treatment. Anti-Mullerian hormone concentration grew while body mass reduced. Conclusion. Minimal 2 kg/m2 reduction is advisable in women with obesity and menstrual disorders.
