BACKGROUND

One of the most important problems of pancreas resection interventions is the occurrence of acute postoperative pancreatitis associated with necrosis of the pancreas stump in combination with anastomotic leakage, the formation of pancreatic and biliary fistulas.

OBJECTIVE

Research of a new method of pancreaticojejunostomy during pancreatoduodenal resection.

MATERIAL AND METHODS

The method of pancreaticojejunostomy during pancreatoduodenal resection was used in 18 patients. The method consists in a certain sequence of carrying out 3 threads parenchyma—duct—intestine—duct—parenchyma in compliance with the suture technology of the duct—mucous type. The period for the formation of anastomoses was, on average, 5—6 minutes. The consistency of the anastomoses, the development of acute pancreatitis during the postoperative period were assessed.

RESULTS

In 17 cases, there were no complications in the postoperative period, including severe acute postoperative pancreatitis. Complications in the form of acute postoperative pancreatitis were observed in one patient (5.5%).

CONCLUSIONS

The developed method of pancreaticojejunostomy is performed with minimal use of atraumatic suture material, which reduces trauma to the pancreatic stump parenchyma. The method simplifies the technique and shortens the formation time of the pancreaticojejunal fistula.
