Резюме / Abstract:

Основным осложнением после панкреатодуоденальной резекции является панкреатический свищ, который регистрируется в среднем у 30% оперированных. Панкреатический свищ значительно ухудшает как ближайшие, так и отдаленные результаты лечения пациентов. Наиболее грозное осложнение — свищ степени C, при котором летальность достигает 35%. В данной работе проанализированы результаты исследований, посвященных изучению факторов риска развития и различным методам купирования панкреатического свища степени C. Выявлены основные факторы и предикторы, которые могут помочь прогнозировать развитие этого грозного осложнения и своевременно принять лечебно-диагностические меры. Рассмотрены различные хирургические методы профилактики и лечения панкреатического свища: окклюзия панкреатического протока, наружная и внутренняя вирсунгостомия, малоинвазивные дренирующие операции под контролем лучевых методов диагностики, релапаротомия с последующей панкреатэктомией, санацией и дренированием брюшной полости, переформирование панкретодигестивного анастомоза. Проанализированы преимущества и недостатки каждого из них. В настоящее время нет единого мнения и алгоритма лечения панкреатического свища степени C. Выбор метода лечения зависит от конкретной клинической ситуации, данных лучевой диагностики и МРТ, находок во время релапаротомии, опыта хирурга и технических возможностей клиники и ряда других причин. Ввиду отсутствия четких показаний к тому или иному объему повторного оперативного вмешательства при панкреатическом свище степени C изучение данного вопроса в настоящее время представляется актуальной задачей.
