Objective — to develop a systematic approach to the single-stage and delayed surgical rehabilitation of patients with primary operable breast cancer (BC), by expanding a range of possible reconstructive plastic surgery (RPS) and procedures that contribute to the achievement of sustainable aesthetic results. Subject and methods. Medical records from 1250 patients (mean age 49.2±0.3 years) with BC have been analyzed since 2013. Patients with Stages I and II BC constituted the largest proportion (40.96 and 38.88, respectively); those with Stages 0, III, and IV BC were 5.12, 14.96, and 0.08%, respectively. There were a total of 630 (50.4%) of the analyzed patients who had undergone RPS aimed at reconstructing or correcting the shape of the breast as an organ and 620 (49.6%) patients who had organ-sparing surgery (OSS). Results. Over the 5-year follow-up period (from 2013 to 2018), the RPS group (n=510) was found to have locoregional recurrences in 10 (1.96%) patients. The progression of the underlying disease was diagnosed in 21 (4.1%) cases. Over the 5-year follow-up period, the OSS group (n=620) had locoregional recurrences in 7 (1.13%) patients (local (n=6) and regional (n=1) recurrences). The progression of the underlying disease was diagnosed in 9 (1.45%) patients. While analyzing the aesthetic results and psychological criteria, the investigators used the questionnaire «Assessment of cosmetic results after organ-sparing surgery, oncoplastic resections, and RPS in BC patients», which is designed at the P.A. Herzen Moscow Oncology Research Institute and the international Breast Q questionnaire to evaluate satisfaction with the result. The mean scores for aesthetic results and psychological criteria were excellent in the OSS group. Conclusion. Surgical rehabilitation of patients with BC contributes to the timely psychological and physical recovery of patients, disability reduction, and a rapid return to their active social life. Oncoplastic resection for BC allows the best aesthetic and psychological results to be obtained.
