Objective: to study the results of surgical treatment in patients with non-small cell lung cancer with tumor ingrowth into the chest. Subjects and methods. Sixty-eight patients with non-small cell lung cancer with tumor ingrowth into the chest have been surgically treated at the P.A. Herzen Moscow Oncology Research Institute since 1980. Histological examination showed that 34 patients had no intrathoracic lymph node metastases (N0), 9 had metastases in the lung root and bronchopulmonary lymph nodes (N1), and 25 had those into mediastinal ones (N2). Surgery as lobectomy with chest wall resection was performed in 50 patients, bilobectomy with chest wall resection in 2, and pneumonectomy in 16 patients. Palliative surgery was done in 7 patients. Results. The 5-year survival rates in patients with N0, N1, and N2 were 41, 18, and 7%, respectively. After palliative surgery none of the patients survived for this period of time. Analysis of prognostic factors can identify the most important ones: the state of the intrathoracic lymph nodes and the type of performed surgery (radical or palliative). Conclusion. The long-term results of surgical treatment for non-small cell lung cancer with tumor ingrowth into the chest in intact lymph nodes are satisfactory: more than 40% of the patients survive for 5 years of a follow-up. Palliative surgery and mediastinal lymph node metastases predetermine a poor prognosis.
