Abstract:

Two-stage liver resections with ligation of one of the portal vein branches are justified in bilobar injury. At the same time, adequate hypertrophy of the contralateral lobe cannot be ensured in 5—20% of cases. The most current and promising procedure for overcoming this obstacle is the so-called in situ split liver resections. In this case, hepatic parenchyma dissection is not delayed (in 5—6 weeks), but immediately during the first operation. The published global experience show that there were a total of about 100 in situ split liver resections as of December 2013 and less than 10 interventions in Russia. In 2013, the Abdominal Cancer Department, P.A. Herzen Moscow Oncology Research Institute, performed two in situ split liver resections in patients with bilobar metastases from colorectal cancer. This paper describes one of the observations.
