Epidermal nevus (EN) is a systematic papillomatosis with inflammatory dysplasia of the upper layers of the skin. A clear classification of EN has not been presented to date. This article presents the classification of EN with the main clinical and pathomorphological features of individual forms. Identification of forms of EN is a complex process requiring the precise algorithm of examination with inspection of the skin, mucous membranes of the mouth and the organs of vision; the examination of the brain. The conclusion of the pathologist, consultation of the neuropathologist and ophthalmologist are necessary. The latter, together with a plastic surgeon, is involved in the therapy of EN, the approach to which is individual. Belonging to the group of EN, verrucose epidermal nevus is a nodular benign developmental defect with abnormal structure. There are its limited and disseminated varieties. In the latter case efflorescences are randomly scattered on the skin, or located in Zakharin—Head zones (more often) or along the nerves and large vessels. Limited form manifests with single dense elements with a wide base and warty surface. The study presents observations of clinical cases of verrucose epidermal nevus, accompanied by pain symptom. The manifestation in a 17-year-old woman occurred at the age of 10 on the site of a postoperative scar, and in a 22-year old one — at the age of 4, and in a 21-year-old man a skin defect developed from birth on the right half of the body. These elements were periodically removed in the clinic by laser and excised with a scalpel, but then they recurred in the same areas and to the same size. Subjective disorders in man were absent. Cases of verrucous epidermal nevus are a rare manifestation of a skin malformation. In two cases, the process involved nerve endings, as evidenced by the pain syndrome
