Aortic stenosis is the most common heart valve disease requiring surgical intervention. Therefore, optimal timing for surgery for aortic valve disease is crucial. Surgery should ideally be performed when the risk of complications exceeds the risk of intervention. Unfortunately, current clinical guidelines substantiate aortic valve surgery in patients with irreversible changes in the left ventricular myocardium. Thus, improvement of diagnostics, optimal timing of intervention and development of surgical techniques are still important. This article reviews the most important aspects of modern guidelines whose changes will contribute to optimization of surgical treatment of aortic stenosis and improve long-term postoperative outcomes.
