Aim — to analyze heart rate variability (HRV) and its effect on potential unfavorable outcomes in elderly patients with acute coronary syndrome (ACS) undergoing coronary stenting. Material and methods. 116 ACS patients who underwent percutaneous coronary intervention (PCI) within 6 hours after pain onset were enrolled. Bare-metal coronary stents were predominantly used. After PCI all patients underwent 24-hour ECG monitoring to evaluate HRV. SYNTAX Score was calculated to assess severity of coronary artery disease. GRACE and CRUSADE scales were used to identify ACS patients without ST-segment elevation and high potential risk of postoperative in-hospital complications. Results. Parasympathetic activity and significantly lower HRV were predominantly observed in elderly ACS patients with ST-segment elevation undergoing coronary stenting (HF 40,3±14,0 mc2 vs 34,0±15,4 mc2; p=0,0113) compared with those without ST-elevation. In last cases activation of humoral-metabolic and sympathetic mechanisms of HRV regulation was revealed (VLF (33,0±16,2 mc2 vs. 26,3±13,1 mc2; p=0,0274) and LFnu (0,54±0,13 mc2 vs. 0,46±0,14 mc2; p=0,047)). Among elderly patients with severe CAD (SYNTAX Score over 22) low HRV was predominant. Increased GRACE and CRUSADE risk was observed in ACS patients without ST-elevation and reduced HRV. Conclusion. Advanced age patients with ACS and ST-elevation undergoing coronary stenting had reduced HRV and predominant parasympathetic effects. Low HRV and LAD damage in elderly patients with ACS and ST-elevation are associated with high risk of complicated in-hospital period.
