28 patients younger than 45 years and 56 patients over 60 years were investigated to evaluate the frequency of application of various reperfusion techniques in myocardial infarction (MI). Levels of souluble Fas-receptor (sFas) and Fas-ligand (sFasL) were detected in serum using ELISA in 2 weeks after MI. Young patients were recurrently investigated after 7.0 (5.7-10.9) months. In 2 weeks after MI statistically significant differences in concentrations of apoptosis mediators were not observed in young and senior patients. Independently from age maximal concentration of sFasL was observed after combined pharmacoinvasive approach (thrombolytic therapy+percutaneous coronary intervention), appearing to be statistically significantly higher compared with the one after percutaneous coronary intervention. Severity of coronary affection and type of reperfusion were the prognostic factors for early postinfarction levels of sFasL. In younger patients increased concentration of sFasl (84 pg/ml and higher) in 2 weeks after MI had prognostic value for new onset of coronary complication in further 6-8 months. After adequate reperfusion (83.3%) during the long period of observation younger patients didn't demonstrate progression of chronic heart failure and presented with decreased level of sFas whereas sFasL level didn't increase significantly.
