Frequency of periprocedural myocardial necrosis after implantation of bare metal stents (BMS) and drug-eluting stents (DES),as well as long-term outcomes were analyzed in patients with coronary heart disease (CAD) after percutaneous coronary interventions (PCI). The trial included 340 CAD patients after planned PCI with implantation of BMS (1 group, n=216) or DES (group 2, n=124). Blood samples were taken prior the procedure and in 8 and 24 hours after PCI for troponin T (TnT) analysis. According to degree of TnT elevation after PCI frequency of myocardial necrosis (every TnT elevation), periprocedural myocardial infarction-type 4A (TnT elevation in 3 times and higher), extensive myocardial infarction (TnT elevation in 5 times and higher) was evaluated. Outcomes were evaluated in 30 days and in long-term period during 30 months in average. The frequency of myocardial infarction after the PCI (35,7% vs 27,4%), periprocedural MI (6,5% vs 3,2%), extended periprocedural MI (4,2% vs 1,6%) was higher for the group with BMS compared with DES group, but did not significantly differ. The first 30-day follow-up for CAD patients was uneventful without development of any adverse cardiac events in the both groups. In case of 2.5 year follow-up, frequency of unfavorable complications was also comparable.
