Abstract:

Aim — the aim the study was to elucidate the influence of metabolic thrombophilia associated with single-nucleotide polymorphism of the methyltetrahydrofolate reductase (SNP С677Т MTHFR) and metabolic syndrome (MS) on the risk of development of the postthrombotic syndrome (PTS) and deep vein thrombosis (DVT) recurrence. Material and methods. We followed 80 patients with metabolic syndrome for one year after an episode of DVT. Among them 58 patients had SNP С677Т MTHFR. At the end of the follow-up PTD we examined patients both clinically and by duplex ultrasound to reveal the signs of DVT recurrence. Villalta score was used to find if PTS developed. Results. Heterozygous variant of SNP С677Т MTHFR did not increase the risk of PTS developement (relative risk (RR) 0.75; 95% CI 0.343—1.642; р>0.05). The risk of DVT recurrence was higher but non-significant in patients with this variant of SNP С677Т MTHFR (RR 3.00; 95% CI 0.780—11.536; р>0,05). The homozygous (ТТ) variant of SNP С677Т MTHFR has been associated with elevated risk of DVT recurrence (RR 6.00; 95% CI 1.693—21.262; р<0.05). The metabolic syndrome increased the risk of PTS (RR 2165; 95% CI 1.194—3.927; р<0.05) but had no influence on the DVT recurrence (RR 0.718; 95% CI 0.356—1.449; р>0.05). Conclusion. The metabolic syndrome significantly increases the risk of PTS development whereas the homozygous (ТТ) variant of SNP С677Т MTHFR enhances the risk of recurrent DVT.
