Abstract:

Goal. Demonstrate the long-term results of treatment of patients with achalasia of cardia III—IV stage by laparoscopic esophagocardiofundoplasty with fundoplication by Dour. Materials and methods. In the work was included 51 patients with achalasia of cardia III—IV stage. The study consisted of two parts: retro and prospective. The analysis was carried out according to fibrogastroduodenoscopy (FGDS), X-ray control, impedance manometry, pH-metry and histological examination of the esophageal mucosa. Results. This operation allowed to stop the dysphagia in 92.16% of cases. The appearance of gastroesophageal reflux was noted in 7.84%. The contraction of the esophagus diameter has been statistically significant confirmed irrespective of the stage and duration of the disease. During the follow-up period, no cases of esophageal cancer have been reported for 14 years. Conclusion. Esophagocardiofundoplasty can be recommended as a surgery of choice in exchange for current cardioplastic operations, including cardiomyotomyby Geller, as well as extirpation of the esophagus in the situation of extensive stage of achalasia of cardia.
