Outcome analysis of treatment of 45 patients with stage I-II peripheral lung cancer is carried out. 22 patients underwent videothoracoscopic lobectomy (VTSL), in 23 patients open lobectomy (OL) was performed. Duration of VTSL was significantly longer than of open lobectomy by 73 min on average (p<0.005). Blood loss during VTSL was lower than open lobectomy and comprised 175±80.26 and 273±103.1 ml respectively. There was no difference in oncologic radicalism between the two groups. No complications were observed after VTSL. In the group of patients after open lobectomy pneumonia, superior bronchial stump insufficiency, chylothorax developed in 3 (13%) patients. Postoperative air leak continued for 2.0±0.53 days after VTSL and 5.8±1.5 days after open lobectomy (p=0.02), time of pleural cavity drainage after VTSL was 6.4±1.86 days and 12±9.2 days after OL (p=0.038). The amount of pain due to VAS in VTSL group decreased to grade 1 by 5-6 days postoperatively and by the time of discharge most patients didn´t suffer pain. The amount of pain in OL group was 5 and more by 5-6 days and by the day of discharge it continued to persist. Videothoracoscopic lobectomy was proved to be safe surgical procedure. Upon that oncologic principle are not compromised. Abovementioned allows performing VTSL as an alternative to conventional open lobectomy for early-stage lung cancer.
