OBJECTIVE

To compare two treatment methods of large common bile duct stones — mechanical and contact lithotripsy.

MATERIAL AND METHODS

605 patients with common bile duct stones were addmitted to Sklifosovsky Research Institute of Emergency Care from January 2019 to April 2022. 53 (8.8%) patients met inclusion criteria for the study — large common bile duct stones (>15 mm in diameter) which could not be removed by a Dormia basket or lithoextraction balloon.

RESULTS

Mechanical lithotripsy was applied to 35 patients, and contact lithotripsy to 18. The efficiency of mechanical lithotripsy was 72%. Complications were noted in two patients. Contact lithotripsy was effective in all cases without any complications.

CONCLUSION

Our study showed 100% efficacy and safety of SpyGlass-guided mechanical and contact lithotripsy in a well-defined cohort of patients with large bile duct stones.
