Currently, both in developed and developing countries of the world, the incidence of liver and bile duct disease is steadily increasing, which makes the problem more urgent. The causes of the initial pathology of the liver, predisposing to the development of liver failure, are diverse — acute and chronic viral hepatitis, drug-induced hepatotoxic drugs, alcohol and drug use, other infectious and parasitic diseases, systemic and oncological diseases, etc. In clinical recommendations, features of preoperative examination and preparation of patients with concomitant liver diseases are presented. The influence of anesthetics on liver function and the effect of hepatic dysfunction on the pharmacokinetics of drugs are discussed. Complications arising in the postoperative period in patients with concomitant liver diseases (hepatic encephalopathy, ascites and spontaneous bacterial peritonitis, hepatorenal syndrome, bleeding from varicose veins of the esophagus, dilution hyponatremia) are considered according to the current recommendations of the European Association for the Study of the Liver (EASL), the American Association for the Study of Liver Diseases (AASLD), the European Society of Anaesthesiologists (ESA), the Russian Society of Surgeons, the Russian Society for the Study of the Liver.
