Нарушения ходьбы при церебральной микроангиопатии: связь с микроструктурной целостностью трактов, обеспечивающих ходьбу

Резюме

ЦЕЛЬ ИССЛЕДОВАНИЯ

Оценить нарушения ходьбы и риск падений у пациентов с церебральной микроангиопатией (ЦМА), их связь с микроструктурной целостностью трактов, обеспечивающих ходьбу.

МАТЕРИАЛ И МЕТОДЫ

У 124 пациентов с ЦМА (STRIVE, 2013) и гиперинтенсивностью белого вещества в стадии Fazekas II—III (средний возраст 62,2±7,9 года) и 30 добровольцев группы контроля (средний возраст 59,8±6,4 года) проведена комплексная оценка ходьбы тестами Тинетти, «6-метровая ходьба», Клинической шкалой тяжести нарушений ходьбы. На основании данных диффузионной МРТ у 80 пациентов с ЦМА и 21 добровольца группы контроля проведена оценка фракционной анизотропии (FA) в лобно-мостовых, кортикоретикулоспинальных и мостомозжечковом трактах методом сегментации.

РЕЗУЛЬТАТЫ

У 68,5% пациентов с ЦМА выявлены нарушения ходьбы, у 41,9% — умеренный и высокий риск падений. FA, отражающая общую микроструктурную целостность, в лобно-мостовых и кортикоретикулоспинальных трактах коррелировала с результатами теста Тинетти (субшкала «равновесие» и общий балл) и последовательно снижалась по мере нарастания тяжести нарушений ходьбы и риска падений. Для наиболее значимых участков трактов (60—80, 80—95) ROC-анализом получены пороговые значения FA, ассоциированные с развитием легких нарушений ходьбы, умеренного риска падений, имеющие высокую чувствительность и специфичность (AUC >0,75).

ЗАКЛЮЧЕНИЕ

Нарушения ходьбы и связанный с ними риск падений являются частыми при ЦМА. Микроструктурные повреждения (по FA) лобно-мостовых и кортикоретикулоспинальных трактов в большей степени на уровне субкортикального и глубокого белого вещества имеют определяющее значение в формировании нарушений ходьбы, риска падений. Полученные пороговые значения FA данных трактов могут быть использованы в качестве объективных маркеров для выявления пациентов группы высокого риска развития нарушений ходьбы и падений на ранних стадиях, что имеет важное значение для своевременной профилактики.
