Assessment of fetal functional status is an integral part of antenatal screening and is especially relevant in detecting fetal growth retardation, which is observed in 10-17% of pregnant women and is a major factor in perinatal morbidity and mortality. Classical Dopplerometry, including assessment of blood flow in the uterine arteries, umbilical artery, middle cerebral artery of the fetus and calculation of the cerebroplacental ratio, has become an accepted test in the last 5 years. The assessment of cerebral hemodynamics, in particular the cerebroplacental ratio, is the main criterion for a pronounced fetal functional impairment. This review article repeatedly emphasizes that changes in intracerebral hemodynamics may not only reflect the current state of the fetus, but also influence the child’s subsequent neuropsychiatric development. This review is devoted to summarizing the accumulated knowledge in the field of cerebral hemodynamic changes in fetal growth retardation and identifying promising directions for the search for new, more accurate diagnostic criteria taking into account modern standards.
