Thrombotic complications are a common pathology in the elderly, representing a serious threat to their health and life. A stumbling block in the work of the dentist with such patients, who are often on prolonged anticoagulant therapy, is hypocoagulation. In the literature there are few studies devoted to the study of small bleeds in the oral cavity of such patients. For the first time, we studied small bleeds in 102 patients who are on various prolonged anticoagulant therapy regimens with rivaroxaban or dabigatran, who contacted a private dental clinic to make an appointment with a dentist-surgeon for a tooth extraction operation. The accuracy of the differences between the indicators of the compared groups was carried out using the Chi-squared test, Fisher’s exact test, and risk ratio calculations. Persons on oral anticoagulants (NOACs) therapy had gingival bleeding in 28.4% of cases and post-extraction bleeding lasting more than 20 minutes in 16.04% of cases. Any of the applied treatment regimens with NOACs leads to bleeding in the oral cavity. The greatest bleeding is caused by dabigatran and its combination with antiplatelet agents, which should be taken into account in preparation for surgical tooth extraction in such patients. The risk of prolonged bleeding after tooth extraction, according to our data, largely depends on the previous occurrence of gingival bleeding.
