Plastic closure of perforation of the nasal septum (NSP) is an important aspect of modern rhinosurgery, which is associated with a relatively high prevalence of this nosology and the complexity of performing surgical intervention. Until now, the problem of choosing a surgical technique and determining the rational timing of intervention in patients with postoperative NSP remains relevant. This is due to the lack of osteochondral support, pronounced cicatricial changes and the complexity of the formation of the mucoperichondrial flap. The current literature does not indicate the actual timing for the closure of postoperative NSP. Traditional guidelines recommend that surgery be performed at least 6 months after previous surgery. A comparative review of different techniques for closing iatrogenic NSP described in the literature was carried out. The author’s own experience and effectiveness of early revision interventions in closing postoperative perforations were demonstrated. The advantage of closing postoperative NSP in the early period due to the absence of adhesions and reducing the risk of rupture between the sheets of mucoperichondrial flaps has been shown. It was concluded that the detection of NSP in the early postoperative period requires early surgical intervention due to the ineffectiveness of observational tactics, the risk of developing adhesions between the mucoperichondrial sheets, which significantly complicates the operation in the delayed period and increases the risk of reperforation.
