The author proposes to hold a discussion on his proposed classification of polyposis rhinosinusitis. The Classification was published in 2003, but there was no discussion of it. The current trend in clinical medicine to identify phenotypes of the disease is in line with the principle laid down by the author of the classification. Polypose rhinosinusitis phenotypes have been identified as the most likely cause of the disease. Much attention is paid to the importance of aerodynamic disturbances in the nasal cavity and perirhinal sinuses. Against the background of this process there are slow immunological and morphological changes that prepare the basis for the formation of chronic rhinosinusitis, including polyposis. The attached external (bacterial or fungal) factor forms a specific clinical form of polyposis rhinosinusitis. The latter, on the background of «biological» defect is considered a general disease of the body. Recommendations on diagnostics of polyposis rhinosinusitis and choice of treatment method according to the proposed classification are given.
