Diseases of the circulatory system are the leading cause of death in the working-age population. Yet, their incidence and prevalence rates in this population remain unavailable to a wide range of specialists since they are not included in the Federal Plan of Statistical Works.
To study regional features of the incidence and prevalence of diseases of the circulatory system in the working-age population of the Russian Federation in 2015—2019.
We estimated incidence and prevalence rates of all diseases of the circulatory system in the working-age population (ICD-10 Chapter IX) and select groups and diseases of this category in the Russian Federation and its constituent entities. We calculated long-term rates for all circulatory diseases, ischemic heart diseases, and cerebrovascular diseases for 2015—2019, their median values and standard deviations. Coefficients of variation were used to assess variability of estimated rates for each constituent entity and of the long-term averages for all entities. The Mann-Whitney test, a non-parametric analogue of the Student’s t-test for comparison of two independent groups, was applied to evaluate the differences in indicators, the statistical significance of which was judged by a p-value ≤0.05.
We observed the annual increase in five (incidence and prevalence rates of all diseases of the circulatory system and those of cerebrovascular diseases, and the prevalence rate of ischemic heart disease) of six indicators under study in the Russian Federation in 2015—2019. A statistically heterogeneous aggregate of annual incidence and prevalence rates was established in 26 constituent entities while pronounced variability was noted in 39 entities of the country. Only in 35 entities of the Russian Federation, annual disease rates demonstrated slight or moderate deviations from the average. We estimated 4.6 to 8.4-fold difference between upper and lower regional disease rates. In most constituent entities, we revealed a statistically significant difference in the rates of diseases of the circulatory system from the corresponding national levels.
Rates of the diseases of the circulatory system in the working-age adult population of constituent entities of the Russian Federation represent a qualitatively heterogeneous aggregate and demonstrate pronounced regional variations, thus necessitating both additional research on the local epidemiological situation and improvement and unification of approaches to diagnostic coding.
