Treatment and rehabilitation of patients with post-traumatic contaminated lacerations present a complex objective due to some unfavorable factors. The concept of delayed multi-stage treatment and rehabilitation is often associated with several reconstructive surgeries, including face transplantation. This is still an unsolved problem due to immunological tolerance of recipient tissues and certain aspects of psychosocial rehabilitation. Advisability of immediate face replantation usually faces the impossibility of this manipulation due to unfavorable conditions for transporting the victim, insufficient organization of assistance at various logistical stages, unpreparedness of senior and junior medical personnel for this manipulation, as well as impossibility of full range of rehabilitation measures in early postoperative period determining the effectiveness and safety of the entire treatment. Various factors influencing the results of early replantation in children are of interest to medical community and encourage specialists to create algorithms for managing patients with posttraumatic subtotal facial defects. Obviously, successful surgical intervention is impossible without proper microsurgical preparation. We present the first experience of posttraumatic subtotal facial replantation in a pediatric patient and discuss the factors of favorable postoperative prognosis.
