Abstract:

The new possibilities of chemotherapy, including neoadjuvant intraarterial chemotherapy, for cervical cancer (CC) have been recently explored. The investigation enrolled 21 women with cervical cancer (сT1bN0M0 — cT3bN0M0). In all (100%) women, the tumors were morphologically presented with grade 1-3 squamous cell carcinoma. All the patients received 2 regional chemotherapy cycles using cisplatin at a dose of 75 mg/m2. On the following day, paclitaxel was systemically administered at a dose of 175 mg/m2. Tumor dimensions decreased in all the patients. The latter showed a clinical response as a reduction of pain syndrome and lack of pre-existing bleeding from the genital tract. Tumor response was evaluated according to the Response Evaluation Criteria in Solid Tumors (REСIST 1.1). Complete and partial tumor regression was recorded in 5 (24%) and 14 (66%) patients, respectively; tumor stabilization was seen in 1 (5%) woman. Tumor progression was diagnosed in 1 (5%) patient. The achieved objective response could subsequently realize a surgical treatment stage in 18 (85%) women; 2 (10%) patients could undergo radiation therapy; due to tumor progression as pulmonary metastases, systemic chemotherapy could be implemented in 1 (5%) patient. Preliminary analysis suggests that superselective intra-arterial chemotherapy in patients with Stage IB1-IIIB cervical cancer can achieve an objective response in 95% of the patients. A surgical treatment stage could be realized in 85% of patients.
