The paper presents a classification that allows distribution of defects after removal of maxillofacial tumors according to their location and complexity categories depending on resected structures and functional abnormalities. The use of the classification makes it possible to choose a necessary bone autograft and to determine the need for uniflap or complex repairs and its staging. The paper gives a clinical example of comprehensive rehabilitation of a patient with a locally advanced right maxillary sinus tumor. After radical combined treatment encompassing radiotherapy and surgical treatment, the patient underwent delayed microsurgical repair of the cover tissues of the oroorbitofacial region, the inferior wall of the orbit, and the right alveolar maxillary process with a musculocutaneous-osseous autograft, including the serratus muscle with the skin area, the eighth rib, and bifid tenth rib. Considering the presence of intraarticular contractures, the right and left temporomandibular joints were replaced using individual endoprostheses. A maxillary obturator and an external orbital prosthesis with a spectacle frame were made for the patient. A combination of microsurgical tissue autotransplantation and innovative maxillofacial rehabilitation technologies could make a full social and functional rehabilitation of the patient.
