Objective: to study the results of radical abdominal trachelectomies (RAT). Subjects and methods. The P.A. Herzen Moscow Oncology Research Institute (MORS) has performed 124 radical trachelectomies since 2008. An abdominal access was applied to 113 (91.1%) patients, 11 (8.9%) were operated on via the vaginal approach. Out of the 113 patients, 98 (86.7%) were found to have a preserved reproductive potential after RAT. The MORS has been carrying out uterine cerclage since 2011. The investigation enrolled 41 patients. Out of them, 11 patients had undergone trachelectomy, 18 had high cervical amputation. Results. Following RAT, 31 patients planned pregnancy. The latter occurred in 10 cases in 9 patients. A reductologist followed up the remaining 22 patients because of infertility. Six out of the 10 pregnancies occurred after in vitro fertilization (IVF). One pregnancy that had occurred spontaneously proved to be ectopic; tubectomy was done. Three cases had miscarriage at 18—22 weeks. After IVF without uterine cerclage, two patients carried their pregnancy up to 28 and 35 weeks. In all the cases, surgical delivery was performed due to preterm amniorrhea. Conclusion. RAT creates conditions for patients’ reproductive function. However, this patient category needs to be further followed up by reproductologists due to their high infertility rates and surgical preparation for a further pregnancy. RAT done simultaneously with uterine cerclage is one of the ways of improving reproductive prognosis.
