Abstract:

Successful treatment of a patient with ruptured aneurysm and chronic DeBakey type IIIb aortic dissection is reported in the manuscript. The first surgical stage was emergency stenting of distal aortic arch and descending thoracic aorta with covering of the left subclavian artery ostium. At the second stage, endoscopic debridement of hemothorax in the left pleural cavity was carried out. Thirdly, endoleak from the left subclavian artery required ligation of this vessel followed by T-shaped carotid-subclavian anastomosis. The patient was discharged after 9 days. Control CT of the thoracic aorta after 3 months confirmed complete obliteration of the false lumen of thoracic aorta, no endoleaks and normal patency of the left common carotid artery and carotid-subclavian anastomosis.
