Redo surgery after thoracic endovascular aortic repair

OBJECTIVE

To study long-term complications after thoracic endovascular aortic repair and their surgical treatment.

MATERIAL AND METHODS

There were 10 cases of redo surgery following primary thoracic endovascular aortic repair. Mean age of patients was 66±11 years. The period between primary TEVAR and redo surgery was 22±15 months. The causes of redo interventions were continued growth of the false lumen of descending thoracic aorta (DTA) due to endoleak type Ib (n=1), thoracoabdominal aorta aneurysm (n=1), DTA aneurysm below the implanted stent-graft (n=1), divergence between two stent-graft modules in DTA (n=1), distal stent-graft-induced new entry (n=1), retrograde dissection of ascending aorta (n=1), stent-graft migration followed by endoleak type I and continued pulmonary bleeding (n=2), secondary aorto-esophageal fistula (n=1), endoleak type Ia (n=1).

RESULTS

Redo TEVAR was performed in 5 patients (50%), open surgery — in other 5 (50%) patients. One patient (10%) died on the 5th day after closure of aorto-esophageal fistula due to multiple organ failure.

CONCLUSION

Redo surgery after previous thoracic endovascular aortic repair may be safely and effectively performed with high survival rate.
