This review is devoted to the algorithm of the diagnosis of coronary artery disease (CAD) with analysis of the possibilities and shortcomings of conventional instrumental techniques and new laboratory research methods. The authors analyzed the prospects for simultaneous use of the well-known invasive and non-invasive (coronary angiography, contrast-enhanced multispiral computed tomography of the coronary arteries, stress-echocardiography, etc.) imaging methods and new non-invasive biomarkers in the diagnosis and stratification of the risk of CAD. Cellular (peripheral blood mononuclear cells, endothelial progenitor cells), biochemical (transforming growth factor beta (TGF-beta1), cell adhesion molecules (CAM), monocyte chemoattractant protein-1 (MCP-1), stromal cell factor 1a (SDF-1a), lectin-like receptor of low-density oxidized lipoproteins 1 (LOX-1), pentraxin 3 (PTX3), etc.), and epigenetic biomarkers (miR-208a, miR-499, miR-133 and miR -1 and others) are analyzed.
