Aim — to analyze the effectiveness of synthetic inhibitor of fibrinolysis (tranexamic acid, TA) for prevention of bleeding after coronary artery bypass surgery. Material and methods. There were 3 groups of patients with coronary artery disease (CAD). Off-pump procedures were applied in group I, on-pump — in group II. TA was used in both groups. Group III (control) was represented by the patients who underwent on-pump myocardial revascularization and TA was not applied. Results. There was greater blood loss in the control group (507±54 ml) compared with groups I and II (210±34 and 275±63 ml, respectively; p<0.05). Hemodilution was followed by reduced values of platelets and hematocrit by the end of surgery in all groups. On-pump procedures were associated with advanced reduction of the level of fibrinogen by the end of surgery in groups II and III (by 33.7 and 34.3%, respectively) compared with group I (22.8%; p<0.05). Fibrinogen content increased by 35.7% in group I, 35.1% in group II and 17.4% in the control group (p<0.05) after 18 hours. Conclusion. TA administration is highly effective prevention of «non-surgical» bleeding after coronary artery bypass surgery regardless the use of extracorporeal circulation.
