Abstract:

Aim - to analyze surgical treatment of patients with aortic valve stenosis and reduced LV ejection fraction. Material and methods. Early outcomes of 103 patients who were operated at the Department of Heart Valve Surgery for the period 2001-2014 were retrospectively analyzed. Inclusion criterion was severe aortic stenosis with reduced left ventricular ejection fraction (LVEF) below 50%. Mean age was 57.1±10.5 years. Patients were divided into 2 groups. Group 1 consisted of 43 patients with severe LV dysfunction (LV EF < 40%). Group 2 included 60 cases of moderate decrease of LV EF (40-50%). Results. In-hospital mortality was 3.9% (4 patients). Causes of death were cardiovascular failure (2 cases) and severe rhythm disturbances (2 cases). Univariate analysis revealed following predictors of death: age( p=0,04), female gender (p=0,002), small diameter of the prosthesis (19 and 21 mm). Conclusion. Left ventricular ejection fraction alone can not be used to determine the indications for surgery in patients with severe aortic stenosis and decreased LVEF. Patient’s physical state and severity of comorbidities should be taken into account.
