It was analyzed the results of surgical treatment of aortic arch aneurysms by using of antegrade brain perfusion in 59 patients. It was detected that risk of neurological complications, frequency of multiorgan failure and death were significant higher in case of  branches of aortic arch and/or kidneys pathology before surgery. It was observed that time of internal organs ischemia during aortic reconstruction should not exceed 30-45 minutes with maintaining of temperature between 24-26 °С. This conclusion is valid in case of adverse prognostic factors presence in preoperative period.
