Abstract:

Results of subcomissural annuloplasty carried out to 44 patients with dilated aortic valve annulus and concomitant 2-4 degree aortic valve insufficiency were analyzed. After 6 and 12 months after the operations all patients underwent transthoracic echocardiography. Stabilization of annulus diameter without development of stenosis or insufficiency was achieved in all patients in both early and long-term postoperative periods. Aorto-right ventricular fistula due to cut of suture in fibrous part of interventricular septum was formed in 1 (2.3%) patient by 2 months after operation. Aortic valve function was still preserved. The patient was re-operated with preservation of native valve. Lethal outcomes were not observed. It is drawn a conclusion that subcomissural annuloplasty for dilated aortic valve annulus and concomitant aortic valve insufficiency independent from valvular pathology allows stabilizing the annulus and preserving the satisfactory function of native valve.
