Angiographic and clinical efficacy of coronary angioplasty for chronic coronary artery occlusions (CCAO) applied using antegrade (1 group, 45 patients) and bilateral (2 group, 32 patients) techniques was studied. Angiographic success was achieved in 35 patients from the 1 group (77.7%). Among them all interventions had direct clinical success. In long-term period clinical success was observed in 29 patients (82.8%) from the 1 group. Increase of left ventricular ejection fraction from 54,8±3,7 to 57,9±3,5% (p<0,01) was estimated. Angiographic success was achieved in 23 patients from the 2 group (71.8%). Among them all interventions had direct clinical success. In long-term period clinical success was observed in 17 patients (73.9%) from the 2 group. Increase of global myocardial contractility from 54,3±3,6 to 56,2±3,6% (p<0,01) was observed as well. It is proved that recanalization of CCAO allows improving of condition in both short-term and long-term periods. It is shown that despite of technical complexity of recanalization using bilateral technique it is clinically effective and can be approved when antegrade approach is impossible and in case of high anesthetic risk or disagree of the patients from surgical treatment.
