63 patients suffering from coronary heart disease with low left ventricle ejection fraction undergoing direct myocardial revascularization were included in double-blind randomized placebo-controlled study. 3 variants of preoperative preparation (levosimendane, dobutamine and placebo) were analyzed. The changes of central and intracardiac hemodynamics were studied using transpulmonary thermodilution and ultrasound echocardiography with pulsed dopplerography in preoperative period. It is shown that levosimendane administration 24 hours before coronary bypass grafting improves the indices of central hemodynamics in patients with left ventricle ejection fraction lower than 45%. It is accompanied by decrease of diastolic dysfunction. Infusion of levosimendane before the operation decreases the frequency of organ failure, duration of stay in emergency unit and postoperative mortality.
