The article presents a clinical case of treating a patient with an esophageal-pleural fistula that developed after pneumonectomy on the right due to lung cancer, accompanied by purulent empyema of the pleura on the right. The patient had a history of installing three partially covered stents to delimit the esophagus from the pleural cavity. The tactics of staged endoscopic treatment of this pathology, which is aggravated by the development of granulation stricture of the esophagus in the area of uncovered parts of stents and dysphagia 3—4 degree is covered. A step-by-step method for removing previously installed partially coated esophagus endoprostheses using an intermediate stenting of granulation esophageal stenosis is described in detail. As a result of staged endoscopic treatment, a persistent positive effect associated with the relief of the phenomena of dysphagia and the delimitation of the digestive tract from the pleural cavity were achieved. It was possible to remove previously installed partially covered stents and to delimit the pleural cavity from food intake. Moreover, at all stages of treatment, it was possible to avoid the imposition of a gastrostomy and preserve the patient’s natural type of nutrition.
