Current surgical treatment strategies for patients with chronic suppurative otitis media (CSOM) and cholesteatoma complicated by a labyrinthine fistula (LF) are as gentle as possible and aimed at preserving inner ear function. The otolaryngologist’s goal is to eliminate the bony labyrinth defect in the most reliable manner.

OBJECTIVE

To improve the effectiveness of surgical treatment of large labyrinthine fistulas in patients with chronic suppurative otitis media.

MATERIAL AND METHODS

Twelve adult patients with CSOM and cholesteatoma complicated by a lateral semicircular canal fistula were treated. The average age of the patients was 49±0.24 years; there were 7 women and 5 men. All patients had a unilateral disease. A new technique for plastic closure of large LFs, the spiked closure technique, was used, which involves forming a dense wall of three layers (cartilage, perichondrium, and fascia) at the defect site.

RESULTS

Following plastic closure of large labyrinthine fistulas using the spiked closure technique, clinical manifestations of cochleovestibular disorders in the postoperative period were significantly reduced.

CONCLUSION

The effectiveness of surgical closure of large labyrinthine fistulas using the spiked closure technique has been demonstrated.
