Chronic obstructive pulmonary disease (COPD) is a serious global public health problem, reflecting the rising trends in morbidity and mortality from smoking-related diseases. Smoking is a major modifiable risk factor for COPD. Modern understanding of the problem suggests that this disease develops in 50% of tobacco users rather than in 15% as previously thought; that COPD can originate even in utero and in early childhood, and the understanding of the mechanisms underlying the disease is extremely important for its timely prevention. Prevention of COPD should be aimed at overcoming tobacco consumption, taking into account all other risk factors for these diseases. Current smoking cessation programs include behavioural advisory support in combination with pharmacotherapy with first-line medications, such as nicotine replacement therapy or varenicline. COPD patients need more care than healthy smokers, and smoking cessation interventions are the most cost-effective measure to reduce the rates of pulmonary function decline and morbidity and mortality from this disease.
