OBJECTIVE

To improve the results of combined treatment in patients with kidney cancer metastatic to the bones of the spine.

MATERIAL AND METHODS

The investigation enrolled 87 patients with kidney cancer metastatic to the thoracic and lumbar spine. The patients’ mean age was 57 (25-76) years. There were 54 (62%) men and 33 (38%) women. Lumbar and thoracic spinal injuries were detected in 56 (64%) and 31 (36%) patients, respectively. A control group consisted of 68 patients who had undergone surgical treatment without preoperative embolization. MReye 0.035" and 0.021" metallic coils (COOK Inc.), 300-500-µm polyvinyl alcohol granules, 100-300-500-µm microspheres, liquid adhesive composition, and their combinations were used as an embolization agent.

RESULTS

In the group of patients undergoing preoperative embolization, the mean intraoperative blood loss was less than that in the control group without embolization. A similar correlation was also maintained when analyzing the duration of the operation. During and after embolization, there was a transient pain syndrome change in the affected area.

CONCLUSION

Preoperative X-ray endovascular embolization is one of the types of minimally invasive interventions, which can effectively reduce intraoperative blood loss and surgery duration in patients with kidney cancer and metastatic lesions in the thoracic and lumbar spine when they undergo decompression and stabilization surgery.
