Objective: to improve the long-term results of cytoreductive treatment in patients with synchronous multiple bilobar hepatic metastases from colorectal cancer, by applying radiofrequency thermal ablation (RFTA). Subjects and methods. A study group included 78 patients having tumors of the colon and rectum with synchronous multiple bilobar hepatic metastases, who in 2007 to 2015 had received surgical treatment (cytoreductive operations to remove their primary colon tumor) in combination with RFTA for liver metastases, by employing a Cool-tip. A control group consisted of 90 patients with tumors of the colon and rectum and synchronous multiple bilobar hepatic metastases, in whom in this period their primary colon tumor was removed without RFTA. Results. RFTA for simultaneous multiple bilobar hepatic metastases during cytoreductive surgery makes it possible to achieve 22.7% of the three-year event-free survival rates and 4.3% of the five-year overall survival ones and to increase the median event-free and overall survival by 9 to 22 and by17 and 29 months, respectively (p=0.000) compared to the non-RFTA group. Conclusion. The use of RFTA for synchronous multiple bilobar hepatic metastases from colorectal cancer during surgery to remove primary tumors does not worsen the immediate results of surgical treatment compared with surgical treatment without RFTA.
