Abstract:

Features of functional mitral regurgitation (MR) in patients with aortic valve disease are analyzed in the article. Patients with aortic stenosis, aortic insufficiency or combined aortic valve disease were enrolled. There were 25 trials and 2737 patients. Moderate MR was under special attention. Aortic valve replacement was performed in the majority of researches, myocardial revascularization — in some cases. The main variable of assessment was MR in early and long-term postoperative period after aortic valve replacement. Predictors of postoperative MR were moderate MR, pulmonary hypertension, tricuspid insufficiency, left ventricular systolic dysfunction, atrial fibrillation, tethering depth of mitral leaflets, diabetes mellitus. The relevance of this problem is confirmed by absent consensus regarding management of concomitant MR in patients undergoing aortic valve surgery. Analysis of predictors of progression/persistence of MR in patients with aortic valve disease can give a new level of evidence in this issue.
