Abstract:

Abstract Aim — to analyze long-term postoperative outcomes in patients with extremely reduced left ventricular contractility and significant mitral insufficiency. Material and methods. There were 63 patients with extremely reduced left ventricular contractility (ejection fraction <30%) and significant mitral insufficiency who underwent effective suture mitral annuloplasty and direct myocardial revascularization. Mean follow-up was 23.9±7.2 months. Results. Incidence of recurrent mitral insufficiency was 43.7%, while fibrous ring diameter was preserved throughout entire follow-up period. Conclusion. Cicatricial alteration of left ventricular wall with contrast accumulation depth over 50% of the thickness of the segment in the projection of posterior papillary muscle according to preoperative MRI was one of significant predictors of recurrent mitral insufficiency.
