Aim. To evaluate an effectiveness of CEE in patients after stroke, to determine neurological dynamics and quality of life. Material and methods. The study included 122 patients after stroke and atherosclerotic lesion of supra-aortic vessels. Patients were divided into two groups: group I — 70 patients after CEE, group II — 52 patients with significant carotid lesion but carotid reconstruction was not performed. Patients were divided into age subgroups: middle age (44—60 years) — IA (n=26) and IIA (n=18), advanced age (61—89 years) — IB (n=44) and IIB (n=34) subgroups. Results. Recurrent stroke occurred in 22.4% and 8.5% in both groups respectively for a 3-year follow-up (p=0.05). Overall recurrent stroke-related mortality was 5% (3) in surgical group and 8% (4) in conservative group. In advanced age patients significant cerebral re-ischemia was in 32% of group IIB and in 9% of group IB (p<0.05). Stroke incidence and mortality rate were significantly lower in elderly patients after CEE: 18% in group IB vs. 45% in group IIB, TIA incidence + mortality were 36% in group IB vs. 45% in group IIB. The number of patients without neurological disorders increased by 15%. The number of patients with mild neurological disorders (walking without assistance) decreased from 8.5 to 2%. Conclusion. CEE is effective to prevent recurrent stroke and to decrease the risk of neurological deficit progression. Surgery is safe and effective in elderly patients.
