Time from symptoms oneset to reperfusion remains important for long-term prognosis and successful treatment in STEMI patients regardless of reperfusion therapy method. Aim. To define factors affecting pre-hospital and in-hospital logistics and to compare the early outcomes in patients who underwent primary PCI with those who received pharmacoinvasive treatment. Material and methods. The study enrolled 259 patients with STEMI who were hospitalized for the period from 01 January 2012 to 01 January 2013. Comparative analysis of safety and efficiency of primary PCI (n=91) and pharmacoinvasive treatment (n=60) was performed. Results. Mean time from first medical contact (FMC) to reperfusion didn’t significally differ in two groups: 146 (110; 180) min vs. 116 (87; 177) min; р=0.7. There was significant difference in mean «door-to-balloon» time: 61 (41.75; 90.75) min  vs. 117 (62.75; 232.5) min; р=0.001. Significant differences in achieving primary and secondary end-points between two groups were absent. Conclusion. The early outcomes didn’t significally differ between two groups. Significantly increased «door-balloon» and «pain-PCI» time in case of pharmacoinvasive strategy allows you to «win» time for PCI and to eliminate possible technical and organizational delays prior to PCI.
