Aim — to evaluate the efficacy and safety of anticoagulant therapy with rivaroxaban for deep vein thrombosis of the upper extremities. Material and methods. A prospective observational trial included patients with the first episode of symptomatic deep vein thrombosis of the upper extremities. Enoxaparin (1 mg/kg twice a day) was initially administered in all patients for 24–48 h with following prescription of rivaroxaban: 15 mg twice a day for 3 weeks, then 20 mg once a day. There were 30 patients: 13 men and 17 women aged 28—78 years (mean age 52.4±17.3 years). The main criterion was symptomatic recurrent venous thromboembolic complication (VTEC). Results. All patients were followed-up during 6 months. Recurrent VTECs were not observed in any (0%) case ( 95% CI 0.0—11.4). Asymptomatic recurrences were also absent. Recanalization of venous segments was achieved in 100% of cases (95% CI 88.7—100.0). Signs of post-thrombotic disease of the upper extremities were noted in 4 (13.4%) cases (95% CI 5.4—29.8). Mean Villalta score was 2.1±1.9. No major bleeding was observed. Clinically significant bleeding developed in 2 (6.7%) patients (95% CI 1.9—21.4), minor bleeding — in 2 (6.7%) patients (95% CI 1.9—21.4). Overall incidence of bleeding was 13.4% (95% CI 5.4—29.8). Conclusion. Treatment of deep vein thrombosis of the upper extremities with rivaroxaban is associated with high efficacy and safety, adequate recanalization and low incidence of post-thrombotic disease.
