To eliminate stones in common bile duct (CBD) during laparoscopic surgeries we suggest applying contact intraductal ultrasound lithotripsy. Original wave guide has been designed 400mm in total length with radially curved at 40° noneffective part 6mm in diameter proceeding into the working section in a conical way 60mm in length and 4mm in diameter. At the end of the working section of the wave guide concave lens 4mm in radius is established. Proposed configuration of the wave guide enables to insert it into CBD lumen through the dilated cystic duct or via choledochotomy incision during laparoscopy and accomplish fragmentation of the stones thus reducing surgical trauma. Optimal parameters of ultrasound for lithotripsy – 26 500—26 700 Hz, generator power output 35, 40 or 65 W – are deduced from experiments. Application of the technique allows avoiding sphincter disrupting surgeries and thus duodenobiliary reflux. In vitro experiments (n=68) showed effectiveness of ultrasound lithotripsy for biliary stones. In vivo experiments (n=20) revealed that application of established ultrasound parameters enables to avoid rough deeply morphological disorders in irradiated soft tissues. Existing superficial changes are completely neutralized within 2 weeks after exposure that speaks for safety of the suggested technique.
