In 2025, the 350^th anniversary of the birth of the Italian doctor Domenico Mistichelli (1675—1717), who is credited with the discovery of the pyramidal decussation, was celebrated. Analysis of translated excerpts from his work «Trattato dell apoplessia» (1709, Rome) reveals that Mistichelli described crosswise fibers in the medulla oblongata, resembling a braid. While his description may only partially align with the modern understanding of the pyramidal decussation, Mistichelli should be recognized as the first to identify the medulla oblongata as the site where descending motor fibers intersect. There is little information about his biography that has come down to our time.
