Objective: to evaluate the hemostatic effect of terlipressin (remestyp) in its intraoperative intravenous use at a jetwise dose of 5 ml in patients surgically treated for uterine fibroids. Subjects and methods. Conservative myomectomy via laparoscopic access was carried out according to the standard procedure. The investigation was conducted in 2 groups: 1) 30 patients in whom uterotonics, but not terlipressin was used during surgery (n = 30) (a control group); 2) 33 patients in whom terlipressin 500 µg was intravenously jetwisely injected during administration of uterotonics (a study group). Basic hemodynamic, general hematologic, biochemical, and coagulation parameters were monitored; the duration of surgery and the volume of blood loss were analyzed. Results. The investigations showed that the mean duration of surgery was 126.8±6 min in the study group and 138.3±5 min in the control group (p<0.01). Intraoperative blood loss was 159.3±16.2 ml and 185.3±25.4 ml, respectively (p<0.01), while in the first days the volume of daily hemorrhagic discharge was lower in the study group (92±55 versus 123±45 ml (p<0.05). The tolerability of the drugs was comparable and did not differ in the groups. Conclusion. Terlipressin (remestyp) is highly effective in preventing intraoperative bleeding, well tolerated by patients, and significantly reduces the duration of surgery.
